We help with the heavy lifting, so your teams can focus on care

Post-acute care teams are exceptional at what they do.
We're here to make sure the infrastructure around them is too.

Built for a problem
we saw up close

Post-acute care was never built to work as a system. It was built for isolated tasks, stitched together with tools that don't share context or coordinate action.

As patients move between hospitals, skilled nursing, home health, and beyond, critical details get dropped, decisions get rushed, and work slips between the cracks. The issue was never commitment; it was coordination.

We built exacare ai to fix that, so care teams can do what they do best.

Backed by world class investors

Laird Russell, Co-Founder & CEO
“Post-acute care teams deserve systems that work as hard as they do. Our goal is to help teams move faster, save time, and focus more on providing the best patient care possible.”

Things we believe

  • Operations should never be the bottleneck for care
The infrastructure of post-acute care should be invisible, fast, accurate, and always working in the background.
  • Post-acute care deserves world-class technology
It's one of the most complex, under-resourced parts of the healthcare system. We're here to change that.
  • The best systems are built with the people using them
We work closely with care teams and operators to understand how work actually gets done. Then we keep improving based on what we learn.
  • AI should be explainable, not a black box
We show our work, surfacing the reasoning behind every recommendation so teams can act with confidence and defend every decision.
